If an institution thinks about switching from a conventional to a high sensitive cardiac troponin assay, I think it’s crucial to consider that you have to get together all key opinion leaders within institution. So in the case of cardiac troponin, the cardiologists, the ED Physiancs, but also the lab experts. Because they need to support the concept, and the concept does not only consist of switching to the new assay, but also to combine it with institution-specific algorithm.

So, a helpful approach might be that you first choose to go to the higher sensitivity cardiac troponin assay, using the 0 hr / 3 hr algorithm, so that you can get familiar with this new assay and get confidence in it. And then later go on to the 1-hour algorithm, because the introduction of high sensitivity cardiac troponin assays improves the safety for the patients, which is more important.
